King Philip Pop Warner requires the following documentation, under the guidelines of Pop Warner Little Scholars and the Hockomock League, our parent organizations: original or certified (raised seal) copy of birth certificate (FIRST YEAR PLAYERS ONLY), copy of birth certificate (RETURNING PLAYERS ONLY), completed and signed registration/medical treatment release form, copy of final report card for the most recent calendar year, a medical eligibility form signed by a physician, which MUST be dated the current calendar year, and a signed Parent Code of Conduct form. Equipment will not be issued, and children will not be allowed to participate in practices until all documentation has been turned in to the team's head coach or paperwork coordinator.
